Programs
Albemarle Home Care administers a number of programs designed to meet the
unique needs of the people we serve.
- Albemarle Home
Health
Provides
skilled nursing, nurse aide, therapy and other health care services in the
home, working closely with and under the direction of the patient's
physician.

Adult Day Health Care
Daybreak, Adult Day Health Care center provides a community-based
program for adults 18 and over who desire companionship or require assistance
during the daytime due to frailty or physical disability.

Albemarle Hospice
Provides comprehensive services to terminally ill patients and their
families. Caring staff members reduce
the intensity of the illness and allow the patient to remain comfortably at
home.
·
Provides
case management services through Medicaid, to meet the personal care needs
of the patient while providing relief to the caregiver/family, designed to
keep the patient at home as long as possible.
