Albemarle Hospice
Since 1983, Albemarle Hospice helps patients and families as they cope with a terminal illness. Hospice isn't about dying; it is about living each day to the fullest and focusing on quality of life. The hospice team teaches the patient and caregivers what to expect in the final stages of life and methods to manage their needs.
DayBreak Adult Day Health Center
Our Adult Day Health Services provide care and support for adults who, due to frailty or physical disability, require assistance during the day. Daybreak provides a range of activities designed to promote social, physical, and emotional well-being. The facility is located in Elizabeth City. Participants may be dropped off by family members, or transportation can be arranged.
Albemarle Bridge Program
The Albemarle Bridge Program is a service to patients and their families in need of education, resources, and support due to life-threatening illness. The goal is to address the patient's current needs while also helping to plan for potential future needs and care.
Community Alternatives Program
for Disabled Adults
Albemarle Home Care serves as the lead agency for the Community Alternatives Program for Disabled Adults (CAP-DA) for Camden, Currituck, Pasquotank, and Perquimans counties. These programs are designed to assist disabled individuals to remain in their homes - and out of nursing homes and other institutions - for as long as possible.
